We have been providing primary care for people living with HIV/AIDS and those at very high risk since 1997. Our program is a team-centered, multi-disciplinary approach that in addition to clinical care, offers access to behavioral health and substance abuse services, a wide range of educational programs, HIV counselling and testing, and youth programs. We provide provides culturally competent, non-judgmental, high quality health care to all patients. A crucial element of providing state of the art health care is the management of data and reporting for our various funding sources which this position will address. The Director of Risk Management in the Department of Quality Improvement is responsible for the day-to-day risk management activities for Philadelphia FIGHT Community Health Centers, including investigating, managing, and analyzing risk management data, complying with risk management related standards and conducting staff training in risk management The Clinical Risk Manager is responsible for the coordination and investigation of serious adverse events (SAE), including potentially reportable incidents to the Board of Directors and provides documentation of all related data and facilitates resolution of reported incidents. They support FIGHT's objective of enhancing and promoting patient safety and quality of care and minimizing loss to protect the assets of the facility. This individual participates in formulating risk reduction strategies, policy and/or organizational changes.
Job Responsibilities

Risk Management: Works in close collaboration with Senior Quality Improvement Advisor, Manager of Clinical Informatics and Clinical Informatics Specialist to coordinate and administer risk identification, investigation, and reduction, monitor risk functions, and perform risk surveys to assess potential risks. Coordinates the risk management program to meet the needs of the organization. Reviews and analyzes all occurrence reports for Risk Management, quality of care, regulatory and insurance concern including tracking all patient safety events reported to the risk management team ensuring full evaluation and completion. Conducts Root Cause Analysis investigations, which includes assessing need for focused review by departments, and/or clinical directors for peer reviews. Maintains risk management information system database. Responsible for monitoring the Safety and Compliance Dashboard within the electronic medical record (EMR) Prepares various incident summary reports; provides all required documentation for investigations, activities and resolutions related to the reported incident (s). Facilitates the peer review process and completion of safety solutions including reviewing all reviews and summarizing trends or other areas of improvement Identifies organizational risk through trend analysis of incidents, incident letters, and reports Acts as a resource person by presenting relevant risk management issues for discussion and evaluation. Keeps abreast of appropriate regulatory requirements related to risk management. Active participation on committees directed toward promoting quality of care and with availability to attend/cover other committee meetings such as Mortality and Morbidity conferences Serves as point person for follow up from cases presented to the Mortality and Morbidity conferences to ensure completion of action steps resulting from discussion Act as lead contact for Closed Point of Dispensing (POD) program through PDPH Assists in developing educational programs/trainings to raise staff awareness to risk exposure and the impact on professional liability. Coordinates risk management activities with health center and organizational leadership to improve outcomes Works with the Departmental Medical Directors and Practice Managers patient safety efforts, and risk management programming. Performs quarterly risk management audits. Prepares reports for organizational leadership and the Board of Directors Accepts other job duties as delegated w/in scope of work Emergency Preparedness:

Maintains and executes FIGHT's Emergency Preparedness Plan Reviews and analyzes existing emergency preparedness and evacuation plans; makes recommendations for modifications to improve safety, efficiency, and effectiveness the plans. Identifies various potential hazards and risks that may arise in the event of an emergency; develops plans to mitigate or manage those risks. Plans, coordinates, and facilitates safety and evacuation meetings and drills as required. Develops, distributes, and maintains emergency communication protocols and documentation, which may include emergency contact trees, emergency response hierarchy, and other records. Ensures that relevant policies and procedures are current and in compliance with applicable regulatory requirements. Monitors and assesses work environment for situations requiring activation of emergency procedures. Directs emergency efforts, collaborating with facility manager regarding partial or complete evacuations and evacuating employees as needed or in the absence of facility manager. Reviews emergency preparedness plans with employees and supervisors; provides updates when employee responsibilities or assigned actions change. Supervises external pandemic response projects, including but not limited to COVID-19 testing and vaccination efforts. This includes: Supervising COVID-19 testing staff Managing relationships with COVID-19 testing sites and vendors In the event of surges in testing demand, aiding with onsite COVID-19 testing operations and offsite patient results delivery Working with the Chief Scientific Officer to modify guidance given to positives in accordance with local and national public health recommendations Collaborating with FQHC staff when additional need for community response occurs
Qualifications/Educational Background: Bachelor's Degree required preferably in a science or health information related field. Master's Degree preferred. Two years' experience in data management required. Risk management experience preferred. Knowledge: Previous experience in science, medicine, health information technology, data entry and database management with strong computer skills (e.g. - Microsoft Access, Word, and Excel). Experience in the use of Electronic Medical Record Systems. Demonstrates the ability to understand the overall purpose of data in a grant-funded setting. ECRI Training required but can be completed upon hire. General: Must be able to work independently and demonstrate good judgment, patience, integrity, organization, and prioritization in multi-task environment. Needs to be detailed-orientated, dependable, and flexible. Ability to manage and consistently meet deadlines. Must be culturally competent and able to work with a diverse, multi-disciplinary team. Commitment to confidentiality of patient health information. Communicate effectively and professionally. Compensation: Competitive with benefits Other Initial probationary period is three months.
